2019 Annual Amenities Survey $250 Sarasota National Account Credit

Only one entry per household. No purchase is necessary. Must be 21 years of age or older to enter. Completed surveys must be received by 5:00 p.m. EST on expiration date of the survey to be eligible to win. One winning entry will be selected in a random drawing of all completed surveys from all participating Clubs. The one (1) winning entry selected will receive the Grand Prize which is a $250 account credit which shall be awarded within 14 days of the random drawing. As a condition precedent to the award of the Grand Prize, the winner shall sign an affidavit verifying his/her acceptance of these terms and conditions and authorizing the use of their name or likeness for purposes of advertising without further compensation or notice. The winner shall be solely responsible for any applicable taxes upon the Grand Prize. For a list of winners, send a self-addressed stamped envelope to, Sarasota National Palm Club, 25500 National Blvd., Venice, FL 34293, to the attention of: 2019 Annual Amenities Survey. In the event of a dispute, the decision of Sarasota National shall be final. Sarasota National reserves the right to terminate or revise this program at any time without prior notice.
